Which form and checklist do you need?
We have developed checklists that you should use when you are assembling your application. These checklists will ensure that you provide us with the correct information when you lodge your applications. This will reduce the likelihood of us having to request further information in order to approve your application.

Licensed Building Practitioner online forms
As of 1 March 2012, any Restricted Building Work (RBW) must only be carried out or supervised by a Licensed Building Practitioner (LBP).
The building consent applicant must use the following forms for registering an LBP, changing the LBP associated with their building consent, or alerting the Department of Building and Housing to faulty work carried out by an LBP.
These forms can only be completed online.

Notification of Licensed Building Practitioner |
Use this form to notify us of a LBP who will be working on a building project. You can submit details of up to 6 LBPs with each form.
This form should also be used if any LBPs are to be added to a building project. |

Change LBP details for a building project |
Use this form to notify us of changes to the details you have already given us about the LBPs who will be working on your building project. For example, if an LBP ceases to be involved, or the work they will be doing changes.
This should only be used if an LBP is to be replaced or removed from a project, not added. |

Report faulty building work |
If you are working on a building project which includes Restricted Building Work and you encounter faulty work, please notify us using this form. |
